If you are enrolled in the MetLife Dental Plan, click here to download and print a dental ID Card. Note: you do not need to present an ID Card to confirm that you are eligible for dental benefits.

Vision

Eye exams are a key part of your overall health – eye doctors can identify early signs of health conditions. Our vision plan allows you to see any eye care provider, but you pay less out of pocket through the Anthem Blue View Vision, Select Network.
